Position: Community Health Nurse
\nReport to: Service Delivery Manager
\nLocation: Bomso (KNUST)- Oforikrom Municipality- Ashanti Region
\nJOB PURPOSE:<\/strong> To organize and provide SRHR services to meet the health needs of the vulnerable group including adolescent girls and young women through the facility and community-based.
\nDuties and Responsibilities
\n1. Assist in developing caseload\/catchment population profile and identify, prioritize and implement programs of care, planning and carrying out SRHR activities
\n2. Provide Sexual Reproductive Health (SRH) information to clients
\n3. Participate in health education activities
\n4. Provide a quality integrated package of essential services including counselling, family planning, child welfare clinics and HIV\/AIDS, STI
\n5. Counsel clients with special problems during outreach and refer to the appropriate facility
\n6. Organize effective and efficient outreach services and ensure timely submission of quality reports
\n7. Assist in ensuring continuity of care by tracing defaulters and making follow-ups visits.
\n8. Assist in organizing periodic community mobilization in the catchment areas
\n9. Liaise with other agencies, like social welfare to assist clients in need of support.
\n10. Prepare and submit written reports as required
\n11. Ensure the formation of volunteer branches in the zone and build the capacities of both staff and volunteers to play their roles
\n12. Protect all confidential information concerning clients obtained in the course of professional practice and make disclosures only with the consent or when disclosure can be appropriately justified.
\n13. Advocate support for programmes from opinion leaders and projects teams for effective implementation
\n14. Adhere to the health and safety policies, guidelines\/protocols (eg. the use of personal protective clothing\/equipment
\n15. Maintain effective communication with colleagues, clients and visitors.
\n16. Carry out other official duties assigned by the supervisor.
\nQualification & Experience<\/strong>
\n\u2022 Valid PIN with Nursing &Midwife Council (NMC) plus Community Health Nurse Certificate.
\n\u2022 Additional midwifery skills will be an added advantage
\n\u2022 A minimum of two (2) years relevant working experience, one of which must be in a similar position.
\nOther Qualities and Requirements<\/strong>
\n1. Must have very good interpersonal, communication, report writing and presentation skills
\n2. Must be honest, transparent, reliable and duty-conscious
\n3. High sense of responsibility, and ability to organise and prioritise a workload
\n4. Must be able to work for long hours due to the workload
\n5. Have excellent human relations skills and be youth-friendly.
\n6. Must be able to work with little or no supervision
\n7. Ability to travel on short notice
\n8. Familiar with GHS Patient Charter, Reproductive and Adolescent Health Policies
\n9. Knowledgeable of Health and Safety
\n10. Have basic computer skills
\n11. Ability to work well in a team setting.
\n12. Understand social, cultural and economic factors involved in SRHR.
